Wellbeing Matters Social Prescribing at Pendleton Medical Centre
Would you like to find out more about what support is available to you to help with your health and wellbeing goals?
Social prescribing is a way to connect people to non-medical support and resources within their community to improve their health and wellbeing. Pendleton Medical Centre has a dedicated Community Connector who can support you with non-medical, lifestyle and social needs.
Your community connector will take time to understand what matters to you. They can help you find local support and activities that are right for you. This may include:
- Other local services to support your health and wellbeing
- To find out more, speak to your GP reception team for a referral to Wellbeing Matters.
- Social and creative activities
- Support to be more active and move more
- Connecting with people who share similar interests or experiences
- Learning new skills or preparing for work at your own pace
- Volunteering opportunities
